ccdf35c657 tdf#139667 (and tdf#107573) chapter/outline corrections 
adjustments to changes made in
   e24e053c82ad4ee86dd7eb4e9490c9c62af2b291 and
   48eee4cf96c555a986ec6b8836373eb15445ff9f

   In some cases, "outline" was incorrectly replaced with
   "chapter".  These are reverted. And in a couple of cases
   the idea of "chapter level" was introduced incorrectly together
   with "outline level". This is also reverted.
