Update Release Notes and ChangeLog

Also, the list of release notes have been removed from the upgrading
document. The referral to the ChangeLog remains. That way, the list
of release notes need not be maintained in several places.

# MariaDB MaxScale 2.1.16 Release Notes -- 2018-04
Release 2.1.16 is a GA release.
This document describes the changes in release 2.1.16, when compared
to release [2.1.15](MaxScale-2.1.15-Release-Notes.md).

The source code of MaxScale is tagged at GitHub with a tag, which is identical
with the version of MaxScale. For instance, the tag of version X.Y.Z of MaxScale
is maxscale-X.Y.Z.
