The evq_length file held the returned number of descriptors from
the last epoll_wait() call. As such it is highly temporal and not
particularly meaningful.
That has now been removed and the instead the average number of
returned descriptors is maintained. That information changes slowly
and thus carries some meaning.
The statistics of the polling system no longer match the implementation it
measures. Modified the statistics to better represent the new system by
calculating the number of epoll events each thread receives.
The polling system now has a concept of messages. This can be used to send
a synchronous message to the polling system which waits for all threads to
process the message before returning.
Currently this is used to flush unused DCBs when server persistent
statistics are reported.
- All now include maxscale/cdefs.h as the very first file.
- MXS_[BEGIN|END]_DECLS added to all C-headers.
Strictly speaking not necessary for private headers, but
does not hurt either.

- First step, no cleanup of headers has been made. Only moving
from one place to another + necessary modifications.
