8ed1ff67c2 Fix avro regression
The fact whether a query explicitly defined a query was ignored by the
statement parsing function and it always assumed the database was
explicitly defined. This caused the TABLE keyword in a CREATE TABLE
statement to be falsely identified as the current database.

7c2a173a17 MXS-1536: Always recreate TABLE_MAP objects
When a table map event is read after an alter table, the old TABLE_MAP
object contains old information. Due to this, as well as the added benefit
of making the code easier to read, the recycling of TABLE_MAP objects was
removed. In practice, there were no benefits to re-mapping the tables to a
different ID.

The ALTER TABLE parsing didn't properly handle escaped values and it used
the current database unconditionally.

764863cb3f MXS-1536: Add Annotate_rows event processing
The Annotate_rows events were not processed which caused the following
table map event to be ignored.

Also removed a false debug assertion. The byte count can be zero and the
pointer is not guaranteed to point to anything valid.

0ca04cb5d5 MXS-1536: Fix crash in CREATE TABLE
If a CREATE TABLE statement had a quoted keyword as the name of a field,
the calculated column count and actual column counts would differ.

In addition to this, oneline comments before the end of the statement
would truncate the SQL due to the fact that the whitespace was squashed
before the comment removal was done.

67386980e3 MXS-1341: Ignore binlog checksums in avrorouter
If the binlog has binlog checksums enabled, the extra checksum bytes are
removed from the end of the event. The avrorouter assumes that whatever
caused the binlogs to appear in the first place already checked that the
checksums are OK.

Also removed one extra byte being added to the length of all query events.

fd77b18c49 Remove false debug assertion in avrorouter
The asserted value can be false without it being an error. When a table is
re-mapped to a different position, there is no guarantee that the previous
value has not been reused by another table.

5c50045227 Fix ALTER TABLE regression in avrorouter
The avrorouter failed to detect ALTER TABLE statements which caused a
regression. Extended the alter table tests to parse the JSON for more
strict validation of test results.

285598e0e5 Fix avrorouter type and name handling
The type and name parsing functions could move outside of allocated memory
as they didn't check for the terminating null character. Also fixed the
printf format string used when the list of used tables is being created.

Fixed CDC testing connector to abort on error and added some extra output
to the cdc_datatypes test.

cbc1e864d9 Use RFC 3986 compliant addresses in log messages
When log messages are written with both address and port information, IPv6
addresses can cause confusion if the normal address:port formatting is
used. The RFC 3986 suggests that all IPv6 addresses are expressed as a
bracket enclosed address optionally followed by the port that is separate
from the address by a colon.

In practice, the "all interfaces" address and port number 3306 can be
written in IPv4 numbers-and-dots notation as 0.0.0.0:3306 and in IPv6
notation as [::]:3306. Using the latter format in log messages keeps the
output consistent with all types of addresses.

37dd561470 Add support for IPv6
Both the listeners and servers now support IPv6 addresses.

The namedserverfilter does not yet use the new structures and needs to be
fixed in a following commit.

eb1e163bdf Only update index file if file is flushed to disk
There's no point in indexing the file if it hasn't been synced to
disk. Any attempts to index the file will fail if the file still has an
open data block.

d61f0d0afd Remove useless debug assertion
The debug assertion assumes that the table definition is always in the
binlogs. If a binlog row event without a table definition is read, debug
versions would crash even though the situation is acceptable and expected.

7d51864402 Clean config.h some more
Moved some typedefs to router.h and server.h, changed a few
constants to these enums. Renamed some types in config.h to
remove "Gateway".

There are still some functions in the public header which are
only used in core, but they seem to fit the theme of public functions
so were not moved.

5b92a1f467 Remove explicit module name from log messages
The module name doesn't need to be logged as MXS_MODULE_NAME will be
automatically added as a prefix to all messages logged by the module.

2cabcea211 Add definitions of MXS_MODULE_NAME to all modules
All modules now declare a name for the module. This is name is added as a
prefix to all messages logged by a module. The prefix should help
determine which part of the system logs a message.

60d4e17996 Allow restriction of module command arguments
The module commands now support an optional flag for arguments that when
enabled checks that the argument module name matches the registered domain
name. This can be used to enforce argument type validity for arguments
that are given to modules that expect objects of a certain type.

For example, this is used by the cache and dbfwfilter to prevent valid
filters but of the wrong type being given as arguments.

a2a38f952a Add [process|thread] [init|finish] functions to modules
The MXS_MODULDE object now contains optinal pointers for functions
to be called att process and thread startup and shutdown. Since the
functions were added to the end, strictly speaking, all structures
would not have needed to have been modified, but better to be
explicit. In a subsequent change, these will be called.

C++ does not support flexible arrays, so for the time being C++
modules are restricted to 10 parameters. Better approach is to
factor out the parameters to a separate array and then just store
a pointer to that array in MXS_MODULE.
