c18f9c6bd7 MXS-2883: Handle AuthSwitchRequest packets
The backend didn't expect AuthSwitchRequest packets in response to the
handshake response packets. This is allowed by the protocol and appears to
happen with at least MySQL 8.0.

6dd4a04c5d cherry-pick: MXS-2803: Write all buffered data
Since the queued queries will never be inspected after the COM_CHANGE_USER
completes, they should all be written instead of only the first packet.

a65220165a Fix use of gw_get_shared_session_auth_info
If the client DCB of the session was passed into the function, it was
possible that the session pointer for it was already set to null. The
session pointer of an open DCB is never null but a client DCB's session
pointer can be null if accessed via the MXS_SESSION object.

484ff06795 MXS-619: Send fake response for unconnected DCBs
This causes the connection failure to be counted as an authentication
failure instead of a connection error. The former never causes the host to
be blocked which effectively solves the problem for most cases. The only
case where this would not work is where the network buffer for a backend
DCB is full right after the connection is created.

e057c751cd Improve handleError error messages
The hangup and error handlers now have unique messages. Although the
behavior in the handlers is practically the same in both cases, the cause
of the error is not the same.

If a socket error is present, it is added to the error message. If an
error is present, it should clearly show the reason why the TCP socket was
closed.

The is_fake_event boolean helps distinguish fake events from real
ones. This makes figuring out the real source of hangup events easier.

40f35618ad MXS-2562: Fix out-of-order error during COM_CHANGE_USER
If an error is generated while a COM_CHANGE_USER is being done, it would
always use the sequence number 1. To properly handle this case and send
the correct sequence number, the COM_CHANGE_USER progress needs to be
tracked at the session level.

The information needs to be shared between the backend and client
protocols as the final OK to the COM_CHANGE_USER, with the sequence number
3, is the one that the backend server returns. Only after this response
has been received and routed to the client can the COM_CHANGE_USER
processing stop.

7db87784ac Deliver hangups only to valid DCBs
If a DCB was closed and a hangup event was sent to it via
dcb_hangup_foreach shortly after it was closed, the DCB would still
receive it even if it was closed. To prevent this, events must only be
delivered to DCBs if they haven't been closed.

5b43940559 Track session state only when required
The protocol should not track the session state as the parsing is quite
expensive with the current code. This change is a workaround that enables
the parsing only when required. A proper way to handle this would be to do
all the response processing in one place thus avoiding the duplication of
work.

7fb2ae571b Improve blocked host error message
The error now explains how the problem can be mitigated by increasing
max_connect_errors on the backend server.

021d48f94c Log low-level reason and idle time on master failure
If the connection to the master is lost, knowing what type of an error
caused the call to handleError helps deduce what was the real reason for
it. Logging the idle time of the connection helps detect when the
wait_timeout of a connection is exceeded.

81937c635b Fix current command tracking during LDLI
When a LOAD DATA LOCAL INFILE was executed, the client and backend
protocols would update the command byte leading to misinterpretations of
the data.

c447e5cf15 Uncrustify maxscale
See script directory for method. The script to run in the top level
MaxScale directory is called maxscale-uncrustify.sh, which uses
another script, list-src, from the same directory (so you need to set
your PATH). The uncrustify version was 0.66.

4da7e4678f Improve protocol level result collection
The result collection now covers more cases, including the use of
COM_CHANGE_USER. The addition of COM_STMT_EXECUTE to the list of commands
that generate result set responses is needed in order for the code to take
the correct branch.

28609a2c77 Remove session command processing from mariadbbackend
With the removal of the old session command implementation, the code that
used it can be removed or replaced with newer constructs. As a result, the
backend protocol no longer does any session command processing.

The three buffer types, GWBUF_TYPE_SESCMD_RESPONSE,
GWBUF_TYPE_RESPONSE_END and GWBUF_TYPE_SESCMD as well as their related
macros are no longer used and can be removed.

c039821467 MXS-1883 Maintenance is now the only user-modifiable bit for a monitored server
The request to turn maintenance off/on is a separate flag, although the actual
status is still contained in the status bitfield.

3f4d6391b5 MXS-553: Add diagnostics_json to protocol API
The protocol now allows protocol modules to return JSON formatted
information about the protocol module internals. Currently this is only
implemented by the mariadbbackend module and it returns the current
connection ID on the backend server.

cdb43335a2 MXS-553: Update and document the protocol API
Removed unused and properly documented all entry points in the protocol
module API. As the removal of the `session` entry point is an backwards
incompatible change, the protocol API version was updated.

d6c44aaf52 MXS-1804: Allow large session commands
Session commands that span multiple packets are now allowed and will
work. However, if one is executed the session command history is disabled
as no interface for appending to session commands exists.

The backend protocol modules now also correctly track the current
command. This was a pre-requisite for large session commands as they
needed to be gathered into a single buffer and to do this the current
command had to be accurate.

Updated tests to expect success instead of failure for large prepared
statements.
