96a477ec89 MXS-2490: Send error to client on unknown PS handle
If a client requests an unknown binary protocol prepared statement handle,
a custom error shows the actual ID used instead of the "empty" ID of 0
that the backend sends.

b0d8535ead Allow master changes at transaction start
When a BEGIN statement is being executed without a master connection but
when one can be created, the BEGIN statement would be treated as if a
transaction was already open. Since the statement only starts the
transaction, it is allowed to be routed to a "new" master regardless of
the transaction statem.

This fixes the failure to start a transaction when lazy_connect is
enabled.

26b2897280 MXS-2456: Cap transaction replay attempts
In most cases it is reasonable to stop attempting transaction replays
after a certain number of failed attempts. This prevents transactions from
being repeatedly replayed on the same server over and over again if, for
example, it keeps crashing.

dd188962cd MXS-2427 Check all hints when routing
Now considers other routing hints if first one fails. The order is inverted compared
to e.g. namedserver filter settings because of how routing hints are stored. If all hints
are unsuccessful, route to any slave.

6421af1bb4 Backport query queue changes to 2.3
Backported the changes that convert the query queue in readwritesplit into
a proper queue. This changes combines both

6042a53cb3 Replace raw GWBUF pointers with mxs::Buffer
Now that the query queue is stored in an actual container, it is only
logical to use mxs::Buffer instead of GWBUF as the stored type.

5e3198f831 Replace the plain GWBUF query queue with std::deque
Using a std::deque to store the queries retains the exact state of the
object thus removing the need to parse the query again. It also removes
the need to split the queue into individual packets which makes the code
cleaner.

0001babd26 Clean up readwritesplit routing functions
Moved the more verbose parts of the routing code into subfunctions and
arranged it so that more relevant parts are closer to each other. Also
added the SQL statement that is being delayed to the message.

4bf9fa872c MXS-2313: Use servers of same rank in readwritesplit
When a readwritesplit session has a connection to a master server, servers
of the same rank as the master are used. If no master connection is
available, the server with the highest rank among all connected servers is
used. If there are no open connections, the server with the best rank is
chosen and a connection to it is made.

Connections with different rank values than what is the current rank value
of the session will be discarded. This reduces the use of server with
different ranks when the master server of a session fails. Without the
active pruning of connections, slave connections to primary clusters
without masters would remain in use even after the primary master
fails. This guarantees full switchover to a secondary cluster if a master
change occurs.

ba448cb12c MXS-2313: Clean up readwritesplit connection creation
The connection creation is now internal to RWSplitSession. This makes the
code more readable by removing the need to pass parameters and allowing
easier reuse of existing functions. The various conditions require to
create connections are now also checked in only one place.

bfc874dea1 MXS-2350: Connect on session command with lazy_connect
If a session command is executed when lazy_connect is enabled and no
connections have been created, a connection must be made. This makes sure
that the session isn't closed and that the client receives a response.

24ea222ed6 MXS-2350: Allow lazy connection creation
The lazy connection creation reduces the burden that short sessions place
on the backend servers. This also prevents the problems caused by early
disconnections that happen when only one server is used but multiple
connections are created. This does not solve the problem (MXS-619) but it
does mitigate it to acceptable levels.

This commit also adds a change to the weighting algorithm that prefers
existing connections over unopened ones. This helps avoid the
flip-flopping that happens when the absolute scores are very similar. The
hard-coded value might need to be tuned once testing is done.

260ce9b8b8 MXS-2300: Add session command pruning
This commit adds a new parameter that, when enabled, prunes the session
command history to a known length. This makes it possible to keep a
client-side pooled connection open indefinitely at the cost of making
reconnections theoretically unsafe. In practice the maximum history length
can be set to a value that encompasses a single session using the pooled
connection with no risk to session state integrity. The default history
length of 50 commands is quite likely to be adequate for the majority of
use-cases.

bf4aa1ab2c MXS-2295: Keep the COM_CHANGE_USER command
If the whole session command history is cleared, the COM_CHANGE_USER is
lost and the connections can end up using different users.

2c95119a3b Replace STRTARGET macro
The macro was extremely unwieldly to update and made the addition of debug
assertions harder. Rewriting it as an inline function makes this possible.

9adbd2f8f0 Cache the local server statistics object
By storing the server statistics object in side the session, the lookup
involved in getting a worker-local value is avoided. Since the lookup is
done multiple times for a single query, it is beneficial to store it in
the session.

As the worker-local value is never deleted, it is safe to store a
reference to it in the session. It is also never updated concurrently so
no atomic operations are necessary.

1fa3b133c7 Make keepalive ping checks more efficient
The code now only checks the need for a keepalive ping once every
keepalive interval. Reduced the number of mxs_clock calls to one so that
all servers use the same value.

0d09b56f58 MXS-2025 RWBackends as a vector of unique_ptr:s
For lifetime management keep RWBackends in a vector of unique_ptrs.
RWSplitSession keeps the unique_ptrs very private, and provides a vector
of plain pointers for all other interfaces.

20b62a3f3d MXS-2025 Change RWBackend usage to a vector of raw ptrs.
This is essentially just a search and replace to change SRWBackend to
RWBackend* and SRWBackendList to PRWBackends, a vector of a raw
pointers. In the next few commits vector<unique_ptr<RWBackend>>
will be used for life time management.

There are a lot of diffs from the global search and replace. Only a few manual
edits had to be done.

1abcbd64bd MXS-2187: Allow multiple transaction retries
By resetting the replay state the transaction replay can start again on a
new server. This allows the replay process work when a master server is
shutting down.

e56372b153 MXS-2141: Retry query on master if it times out on slave
With causal_reads enabled, the query would return with an error if the
slave was not able to catch up to the master fast enough. By automatically
retrying the query on the master, we're guaranteed that a valid result is
always returned to the client.

ddd6feff69 Move transaction management into a subfunction
The readwritesplit transaction management was a large part of the
clientReply function. Moving it into a separate function clarifies the
clientReply function by hiding the comments and details of the transaction
management.

c447e5cf15 Uncrustify maxscale
See script directory for method. The script to run in the top level
MaxScale directory is called maxscale-uncrustify.sh, which uses
another script, list-src, from the same directory (so you need to set
your PATH). The uncrustify version was 0.66.

f975035e53 MXS-2013 Move or remove STR-macros
- The ones that were not used were removed.
- The ones that were used were moved close to the actual type.
  In most cases some values were missing and if the definition is
  close to the type there is a remote chance that they will stay
  in sync. If detached, they surely will not.

c01840ffb3 Remove unnecessary SConfig from readwritesplit
The configuration doesn't need to be contained in shared pointer as each
session holds its own version of it. This removes most of the overhead in
configuration reloading. The only thing that's left is any overhead added
by the use of thread-local storage.

f3c84d84c7 Fix transaction migration
The transaction migration in the case of a changed master never worked as
transaction replay would only be triggered when the master fails. To cover
this case, the transaction replay just needs to be started when the need
for a transaction migration is detected.

To help diagnose the behavior, the Trx class no longer logs a message when
a transaction is closed. This is now done by readwritesplit which has more
knowledge of the context in which the transaction is closed.

Moved transaction statistics calculations into a member function and
placed all target type specific processing into their respective
functions.

Also inverted the connection keepalive check to also cover hinted queries.

0614ff4c9d Fix handling of transactions with large results
If transaction replaying was enabled and a result was returned in more
than one call to clientReply, a NULL value would be added to the statement
which in turn would trigger a debug assertion.

Similarly any following statements in the transaction would be executed
regardless of whether the result was complete.

Renamed the statement execution function to better describe what it does.

Extended the basic functional test case to cover this.

bd4be3a97b Use shared configurations in readwritesplit
By using a shared pointer instead of a plain object, we can replace the
router configuration without it affecting existing sessions. This is a
change that is required to enable runtime reconfiguration of
readwritesplit.
