Formatted all sources and manually tuned some files to make the code look
neater.

Uncrustify maxscale
...
See script directory for method. The script to run in the top level
MaxScale directory is called maxscale-uncrustify.sh, which uses
another script, list-src, from the same directory (so you need to set
your PATH). The uncrustify version was 0.66.

Take -Wall into use for tests

Take almost all warnings into use except `format-overflow` and
`unused-function`. format-overflow needs to be handled in a separate
commit.

MXS-1717: Fix test regressions

Due to the changes done for MXS-1717, the bug673 test had to be adjusted
and a newline has to be printed after users_diagnostic is called.

Fix 2.2 test results

The changes to object naming and tee filter rewriting caused a number of
changes in the expected outcome of tests and how the tests have to behave.
