           - remove irrelevant <comment>
           - remove colspan="" rowspan="" from <tablecell>
           * change icon sizes to 1cm
           * update to <h1>

        ( swriter/guide/table_sizing.xhp )
           * update to <h1>,<h2>,<h3>
           * change icon sizes to 1cm

        ( swriter/guide/ruler.xhp )
           - remove bookmark to adjusting cell widths
               (because does not appear on this page)
           - remove irrelevant <comment>
           * update to <h1>,<h2>
