Seth Chaiklin b6ece13cb5 tdf#131036 add note on "rename" and "delete" and update gradient help 
-added <note> about "rename" and "delete" option on Gradient
   -move "Add" and "Modify" together with Gradient and <note>
   -change to <h1>, <h2>, <h3>
   -removed point about "loading gradient lists"
   -Add headings and explanations for "Gradient" and "Options"
   -update option names "To Color" and "From Color"
   -update "Preview" label
   -add "Apply" option (for Writer)
